Organic LIVESTOCK
Initial Applicant
All livestock applicants must also be certified in the crop scope. In addition to the required crop scope forms, the following forms are required for first-time livestock applicants:
Organic Certification Application
: Includes general operation information, history of organic certification, if any, and products to be certified. Required for all scopes every year.
Fee Schedule for Organic Certification
: Outlines annual certification fees for each scope, payment worksheet and instructions for payment.
OSP for Livestock Certification
: Details the organic production plan for livestock certification, including natural resources management, feed sources, and living conditions maintenance.
Livestock Herd/Flock List
: Lists all animals requested for certification and their information, including tag numbers, names, and birth dates. List is updated annually.
Materials Input Listing (MIL)
: Lists all inputs used in crop, livestock, or processing scopes, their purpose, and current allowed status. Required for initial applicants and updated as needed when new inputs are added.
Pasture Log
: Lists the days of the year that animals were out on pasture.
Depending on your operation, some of the following forms may also be required. Contact KDA Organic Program (
organic@ky.gov
) to determine what additional forms you may need to submit.
Additional Contact Information
: Authorizes an additional contact person, other than the applicant, for the operation to communicate with KDA if desired.
Off-Farm Manure/Bedding Verification
: Verifies by the seller that manure or bedding purchased from an off-farm location has not been treated with any prohibited substances.
OSP for Livestock Addendum - Feed Rations
: Details feed rations for ruminant livestock and calculates dry matter intake from pasture. Required for ruminants.
OSP for Poultry Certification – Update
: Lists any updates made to the operation’s existing poultry Organic System Plan in the calendar year (if applicable).
Synthetic Facility Pest Management Agreement
: Allows use of synthetic pest management inputs under special circumstances.
Voluntary Surrender of Organic Certification
: Allows operations to completely surrender their organic certification status or an individual scope with this form. Operations may apply again in the future for certification in the previously surrendered scope.
Voluntary Withdrawal Form
: Allows operation to withdraw their organic certification application prior to certification.
Renewal
The following forms are required every year for renewal of livestock organic certification. Forms and fees are due by January 15th annually. If additional changes have occurred in your operation, other updated forms may be required.
