Organic PROCESSING & HANDLING
Initial Applicant
The following forms are required for Initial processing & handling applicants:
Organic Certification Application
: Includes general operation information, history of organic certification, if any, and products to be certified. Required for all scopes every year.
Fee Schedule for Organic Certification
: Outlines annual certification fees for each scope, payment worksheet and instructions for payment.
OSP for Processing and Handling Certification
: Details the organic production plan for processing and handling certification, including product flow through the facility, packaging and labeling, and recordkeeping.
Master Product Listing
: Lists all products produced by the operation that are requested for organic certification.
Materials Input Listing (MIL)
: Lists all inputs used in crop, livestock, or processing scopes, their purpose, and current allowed status. Required for initial applicants and updated as needed when new inputs are added.
Organic Fraud Prevention Plan
: Describes procedures used to verify organic integrity of certified product.
Organic Product Profile
: Lists all ingredients for each product requested for certification, the organic status of the ingredient, and the percent by weight or volume to determine the organic status of the product.
Depending on your operation, some of the following forms may also be required. Contact KDA Organic Program (

) to determine what additional forms you may need to submit.
Additional Contact Information
: Authorizes an additional contact person, other than the applicant, for the operation to communicate with KDA if desired.
Commercial Availability
: Describes nonorganic and/or nonagricultural ingredients used and the operations plan to search for commercial organic options. Form must be completed for each ingredient requested for use.
Commercial Availability Search
: Documents the producer’s commercial availability search for nonorganic ingredients requested for use. Must accompany the Commercial Availability Form.
Nonorganic Ingredient Declaration Processing
: Verifies that nonorganic ingredients used in processing and handling have not been produced with methods prohibited by the NOP. Form is completed by the manufacturer of the ingredient.
OSP for Processing and Handling - Crop Addendum
: Details the organic production plan for any on-farm processing, such as washing and packaging produce.
OSP for Processing and Handling - Livestock Addendum
: Details the organic production plan for the processing and handling of livestock meat products, such as a slaughterhouse.
Private Label Agreement
: Allows processors/handlers to co-pack for noncertified private label companies.
Processor/Handler Ingredient List
: Lists all ingredients planned for use for all products under
Synthetic Facility Pest Management Agreement
: Allows use of synthetic pest management inputs under special circumstances.
Voluntary Surrender of Organic Certification
: Allows operations to completely surrender their organic certification status or an individual scope with this form. Operations may apply again in the future for certification in the previously surrendered scope.
Voluntary Withdrawal Form
: Allows operation to withdraw their organic certification application prior to certification.
Renewal
The following forms are required every year for renewal of processing & handling organic certification. Forms and fees are due by January 15th annually. If additional changes have occurred in your operation, other updated forms may be required.
