Type of License:	CEM (Contagious Equine Metritis) Quarantine for Mares (aged over 731 days)
Licensing Period:	Mares are quarantined upon arrival in Kentucky and remain under quarantine until testing requirements have been met and KDA issues a Quarantine Release Form
Fee Info:	Application to Import and Premise Inspection Fee - $100 per application to import mares. $120 charge assessed per mare received at quarantine facility during regular hours. Additional overtime charge of $60/hr with a minimum of 2 hours apply for mares arriving on weekends or between 5:01pm and 6:59am. Overtime charge of $120/hr with a minimum of 2 hours apply for mares arriving on state recognized holidays.
Requirements:	Application (typically VS Form 17-129) submitted by equine import broker. Inspection of facility by KDA Personnel and completed Import agreement signed by agent of quarantine facility. Completion by approved veterinarian of all prescribed testing and treatment listed on CEM Worksheet, Palpation letter signed by veterinarian (these documents are issued along with Notice of Quarantine).
